Many inconsistencies and confusion in the reporting of KPIs can occur throughout an organization:
Often there is a presentation of KPI and high-level organizational performance reports in leadership and other status meetings to describe how a KPI or a performance measurement from the output of a process is performing.
A status-meeting reporting format may be a table of numbers, an excel spreadsheet, or a red-yellow-green scorecard. However, these reporting formats do not separate process-output-responses common-cause from special-cause variation and do not typically result in the best actions or non-actions to undertake.
Typical formats report the status of Y-responses at one point in time, where there are comparisons to a previous time or a goal. This form of reporting leads to Y management, resulting in unhealthy, if not destructive, behaviors.
